SUNY Upstate Medical University Social Worker - Outpatient Neurology Clinic in Syracuse, New York

Job Summary:

The Department of Social Work at Upstate is currently looking for a Social Worker to join their team. The Social Worker in this position provides comprehensive psycho-social management and support to at risk patients and families. Primary assignment will be located at UHCC serving the neurology clinic, with additional duties as assigned. The social worker will provide psychosocial consultation through assessment, counseling, intervention, planning and referrals to meet the psycho-social needs of patient and families who are experiencing neurological disorders. Candidate will learn to execute Health Care Proxies and have discussions about DNR/DNI, MOLST (eliciting the patient's values) and other end-of-life decisions. Additionally, the social worker will manage patients through collaboration, coordination, monitoring, evaluating opinions and identifying resources to meet the patient's needs. Candidate must possess communication skills, knowledge of community resources and document all interventions in the patient's electronic medical record. Social workers will participate in staff meetings, appropriate in-services, education, and quality improvement activities.

Minimum Qualifications:

L.M.S.W., L.C.S.W. or eligible, 3 years relevant post-master's experience (preferably in a hospital and/or healthcare environment) required. New M.S.W. graduates who are L.M.S.W. eligible will be considered for appointment to entry level (SL2) position at a commensurate salary after obtaining a Limited Permit. Incumbent must obtain L.M.S.W. within 12 months of receipt of limited permit.

Work Days:

Monday-Friday, 8:30 am -5:00 pm, occasional on-call/holiday coverage on rotation based on operational needs.
