Job Purpose:

This is a professional position that is responsible for the care and treatment of adults living with a mental illness. The Case Manager and client will develop and implement the treatment plan, which will include the necessary follow-up, case management and intervention as required.

Essential Job Functions:

  1. Develop a treatment relationship with consumers and formulate treatment plans on his/her caseload that will detail treatment to be provided.

  2. Provide psychosocial rehabilitation, symptom monitoring & management and supportive counseling to consumers on caseload and participate in “team case management”.

  3. Monitor client’s psychiatric medication regime and compliance as directed by the physician.

  4. Provide crisis intervention and supportive counseling to consumers who are in crisis.

  5. Responsible for maintaining treatment-oriented environment (under supervision of program supervisor) and assure consistency in carrying out treatment objectives.

  6. Coordinate utilization of various community resources, day programs and outside health care providers.

  7. Assists clients in financial planning and development of income resources.

  8. Coordinate consumer access to all entitlement programs and will act as protective payee for consumer’s Social Security benefits when appropriate.

  9. Provide treatment related education and liaison with client’s family and natural supports.

  10. Assist client in securing adequate housing and prompting and training for skills necessary to keep housing.

  11. Responsible for coordinating the efforts of all professionals and paraprofessionals involved with each client.

  12. Responsible for formulation of appropriate discharge plan with assistance of client and program supervisor.

  13. Responsible for accurate documentation and maintaining case file as required by agency policy, government regulations and applicable local, state, and federal codes.

  14. Responsible for meeting department designated monthly billing quotas, EHR note entry within 24 hours, biannual treatment plans and other funder required documentation.

  15. Must be able to assist clients with activities of daily living, such as shopping, cleaning, and cooking.

  16. Attends and participates in staff meetings, in-service training seminars and conferences as required.

  17. Responsible for adhering to agency and Milwaukee County (funder) policies and procedures while carrying out daily responsibilities.

  18. Providing one-on-one client coverage as assigned by supervisors and/or psychiatry team.

  19. Attendance at community-based meetings as assigned including, but not limited to, MC3 Change Agent meetings

  20. Responsible for the safety of clients in both normal program settings as well as a variety of settings in the community.

  21. Distinguish where clients who are non-communicative need personal care.

  22. Works in partnership with the quality department to develop appropriate measures in support of program outcomes.

  23. Participates in department quality planning activities.

  24. Position requires participation in reoccurring ‘on-call’ duties and flexibility in daily work schedule to adequately meet program needs, which may include evenings or weekends

  25. Valid driver license with reliable transportation and vehicle insurance that meets agency standards

Required Education, Experience, Certifications, Licensure and Credentials:

Master's Degree is required in the field of Social Work, Counseling, or related Human Services Field.

Minimum Required Education:

Master’s Degree

Minimum Required Experience : Prefer, but not required, 1,000 hours of post-graduate experience working with adults with severe and persistent mental illness.

Required License/Certification/Registration:

Issued By Governing Body: no license required

Travel outside of office (More Than 10%) : Yes

Required Auto Insurance and Valid Driver’s License:

Valid driver license with reliable transportation and vehicle insurance that meets agency standards

Knowledge – Skills – Abilities:

Experience with individuals with dual diagnoses is preferred.

Experience with motivational interviewing is preferred.

Experience of one (1) year working with adults with severe and persistent mental illness is preferred

Physical Requirements, Visual Acuity, and Work Conditions:

Reaching, Extending hand(s) and arm(s) in any direction. Standing for sustained periods of time. Walking, moving about on foot to accomplish tasks, using steps, pushing, pulling. Lifting, raising objects from a lower to a higher position or moving objects horizontally from position to position. Fingering, picking, pinching, grasping items, typing, or otherwise working.

Talking, expressing, or exchanging ideas by means of the spoken word. Hearing, perceiving the nature of sounds at normal speaking levels with or without correction.

Employee is required to have visual acuity to determine the accuracy, neatness, and thoroughness of the work assigned or to make general observations of facilities or structures

Working Conditions, Employee is subject to both environmental conditions: Activities occur inside and outside.

Medium work: Exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.
