Overview

Responsible for the timely coordination of quality behavioral health and substance abuse treatment services for plan membership in a cost-effective manner to promote positive outcomes. Performs utilization and care management for members with behavioral health needs and substance abuse disorders by assessing, implementing, coordinating, monitoring, and evaluating plan of care designed to optimize member’s treatment across the continuum. Ensures member access to services is medically necessary and appropriate.

Responsibilities

  • Reviews service requests for medical necessity and appropriateness.

  • Assesses members primarily through phone calls with occasional visits to members with development of plans to meet their needs.

  • Provides referrals for brief and immediate crisis intervention (i.e. Adult Protective Services).

  • Conducts investigations and holistic assessments to include the member’s physical, mental and psychosocial state.

  • Coordinates community resources for provision of basic services.

  • Provides appropriate service referrals for members and support for their families and caregivers.

  • Works with internal and external multidisciplinary teams in service planning and making referrals.

  • Maintains accurate records and files related to work performed.

  • Other duties as assigned.

Qualifications

MINIMUM EDUCATION REQUIREMENTS

  • Licensed mental health professional in the State of Virginia.

  • Master’s degree in one of the following fields (social work, psychology, psychiatric rehabilitation, sociology, counseling, vocational rehabilitation, human services counseling) with at least 1 year substance abuse treatment experience, OR​

  • RN with at least one year of substance abuse treatment experience

SPECIAL KNOWLEDGE AND/OR SKILLS

  • Ability to work and communicate with diverse cultural groups

  • Substance abuse treatment service delivery system in Virginia

  • Demonstrated ability to work effectively in multi-disciplinary teams

WORK BACKGROUND/EXPERIENCE

  • Minimum of 1 (one) year experience providing direct services to individuals with substance abuse disorder
